Priority Health Conditions
We offer specialised support for patients in four priority health areas - Mental Health, Cancer Support, Perinatal Support and Respiratory.
Our Health Team will give patients the advice and support they need to stabilise their finances, and connect with life-enhancing community services and activities.
Perinatal Support

Who can refer?
This project accepts referrals from midwives, health visitors, neonatal nurses, the perinatal mental health team and family nurse practitioners, and we are working alongside colleagues from Liverpool’s children’s centres.

Who can be referred?
Our health team supports disadvantaged families with young children.

Mental Health
Who can refer?
Our health team works with ward staff and community mental health teams to encourage referrals.

Who can be referred?
Our dedicated team supports mental health inpatients and outpatients across Liverpool.

Cancer Support
Who can refer?
This project team works with Specialist Cancer Support Services and is supported by Macmillan Cancer Care and NHS Liverpool.

Who can be referred?
Our team supports people living with cancer (PLWC).

Respiratory Care
Who can refer?
This project encourages referrals from specialist pulmonary rehabilitation and respiratory services who are treating people with acute breathing conditions.

Our health team supports patients with a diagnosed respiratory condition.
